Documentation: remove duplicated words

Browse Source 
Remove many duplicated words under Documentation/ and do other small
cleanups.

Examples:
        "and and" --> "and"
        "in in" --> "in"
        "the the" --> "the"
        "the the" --> "to the"
        ...
