Anirban Chakraborty b1fc6d3cfa qlcnic: Cleanup patch 
1. Changed adapter structure to move away from embedding hardware and
    receive context structs and use pointers to those objects
 2. Packed all the structs that interface with FW
 3. Removed unused code and structs
